By initialling above I hereby waive, release and discharge the Summer Pro League LLC and its staff, Phoenix Country Day School, City of Tucson, and all sponsors thereof from any and all liability, actions, suits, claims and demands whatsoever for injury, illness or death resulting from participation in the Summer Pro League.
